Item 1. Where are we now: Annual Health Checks and Health Action Plans
- You Said
- How is communication effective around the annual health checks?
- Response
- We have worked hard to make things accessible and various streams to improve communication and reasonable adjustments around GP practices.
- You Said
- Mental health trusts are doing their own separate piece of work looking into AHC’s.
- Response
- It’s all part of a system wide approach with consent of GP surgeries and why some individuals do not an annual health check.
- You Said
- When I’ve gone to have my AHC and taken in my form, they have just recycled it and not given it back to me.
- Response
- We know, we are working on that. Some GP’s don’t realise people want them back.
- You Said
- Some people are finding it hard to get appointments with e-consults or phoning up and having to wait a long time on hold which is difficult for people with learning disabilities.
- Response
- There are pockets with this problem, so this shows that AHC’s are important.

- You Said
- Did you have to have training to be a peer educator?
- Response
- There has been training and a lot of practice when putting the workshops together until gained enough confidence to host workshops.
